某院围手术期Ⅰ类切口手术应用抗菌药物调查分析Investigation in the anti-bacterials usage in treatment of type Ⅰ incision operation in the perioperative periods

摘要(Abstract)：
目的了解围手术期Ⅰ类切口手术抗菌药物现状及合理性。方法采用回顾性调查的方法,随机抽取某院2011年3～6月4个科室8种Ⅰ类切口手术和1种介入治疗的109例患者病历,对围手术期Ⅰ类切口手术抗菌药物使用情况进行调研、分析。结果 109例手术患者中,抗菌药物预防性使用率达97%;平均疗程9.4 d,其中,术后用药时间≤24 h者3例(2.86%),术后用药时间≤72 h者17例(16.00%);预防用抗菌药共11种,一代头孢菌素类占8.0%,二代头孢类占42.0%,三代头孢类占24.0%,其他类占21.9%。结论某院围手术期Ⅰ类切口手术应用抗菌药物基本合理,但仍存在指征掌握不严、选药针对性不强、给药时机把握不科学、术后疗程过长等诸多问题,亟待进一步规范管理。
Objective To investigate the status and rationality of antibiotics usage in treatment of typeⅠclean surgery in the perioperative periods.Methods In the retrospective study,109 patients,with eight types of Ⅰ incision and one interventional therapy,were selected randomly from four clinical departments of March to June 2011.Results In 109 cases,the utilization ratio of antibiotic was 97% and the average treatment was 9.4 days,3 and 17 out of 109 cases within 24 h(2.86%)and 72 h(16.00%)after postoperative treatment,respectively.Eleven different anti-bacterials were utilized,including the first-generation cephalosporins(8.0%),the second-generation cephalosporins(42.0%),the third-generation cephalosporins(24.0%)and the other antibiotics(21.9%).Conclusions The usage of anti-bacterials is basically rational in the period of perioperative.But some irrational aspects in the treatment of Type Ⅰ incision in the perioperative periods should be corrected further according to the standardized management,such as the strictness of medication indication,pertinence in the choice of variety,medication occasion,medication duration and so on.
